Abstract

This chapter will address a topic of considerable importance that is largely unaddressed by medical and health professionals, i.e., Is there a Human Fetal Anesthesia Syndrome? Subtle and persistent adverse effects of early general anesthesia (GA) exposure are well-documented in animals across species. Whether adverse effects of GA can be documented in GA exposed young humans and persist over an individual’s lifespan is both controversial and relatively unstudied. Discussion begins with a review of extant cross-species non-human data regarding GA exposure during respective critical growth spurt developmental periods. The short- and long-term adversities on an animal’s developing central nervous system will be reviewed and summarized. The conversation then extends to what is known about GA exposure during the highly vulnerable human in utero period and early child development. Associations between GA and attentional problems, learning deficit, neuropsychological deficit, and neuropsychiatric disorder in humans yet evidence of causality remains lacking due to methodological weaknesses and multiple confounders as well as limited human investigation. Nonetheless, recent results raise significant concerns about heightened risk and have implications for current anesthetic practice. Neuropsychologists are uniquely positioned to make substantial contributions to conceptualization, methodology, and interpretation in studies of human exposure to general anesthesia (GA). A ‘developmentalistic’ way forward for neuropsychologists will be presented in support of application of innovative technologies informed by the extensive preclinical data showing cellular, synaptic, and neural circuitry disruption during critical growth periods and short- and long-term neurobehavioral effects.
